Qualifications: Must possess high school diploma or equivalent, or have 1 year of experience in this field; ability to read, write and understand the plan of care; must be able to work without direct supervision and demonstrate initiative, empathy, tolerance, consistency and motivation when working with recipients.
Essential Job Functions: Direct Care 1. Assist recipient with all areas of daily living. 2. Provide services outlined in Plan of Care. 3. Advise Service Coordinator of problems. 4. Report violations and abuse allegations, to Service Coordinator. 5. Document daily activities (basic writing skills required) 6. Maintains recipient’s confidentiality. 7. To comply with rules/regulations according to agency guidelines, state and federal licensing and certification requirements. 8. To practice preventative safety and health techniques.
1. Conduct a professional attitude with clients and other stakeholders 2. Abide by the code of conduct and ethics.
Education: High School diploma (includes the years of attendance and graduating year), GED (include year obtained GED), OR Certificate (include year obtained certificate and where certificate was obtained.)
Experience: List 3 places of employment with one of those showing one year of experience working with the mentally or physically disabled and/or tge elderly. Please also include dates of employment (example 09/2008-09/2010). Also list the duties performed while working for that employer.
